The Bragg Creek Medical Practice is now closed. To request a copy of your chart, please note that a release of information form needs to first be signed and all the applicable fees paid. You may request a release of information form from your new family physician and mail/fax it to us, or you may request that we send you such a form by mail. Please note that no charts will be released until all the fees are paid in full.Please be also aware that, due to the volume of requests we are currently receiving, it may take up to 2 weeks for us to answer.

We do not work on evenings or weekends. We will not respond to Requests during the Holidays (December 21 to January 3rd, inclusively).
